Kelly Baxter, MS, APRN, ACHPN is the Lead Palliative Care Nurse Practitioner for Care New England, an integrated health care delivery system located in Rhode Island. With over 17 years of nursing experience, including 6 years as an advanced practice nurse in the field of palliative and hospice care, Kelly has been instrumental in the development of a robust inpatient palliative care consult service across the health system.
Ms. Baxter has been a featured presenter for Healthcentric Advisors (QIO), on care of the seriously ill and dying patient in acute care and long term care settings. She was a guest speaker at the 2013 Senior Health Annual Forum, presenting in conjunction with faculty from Institute for Health Improvement regarding “The Conversation Projectâ€, a national campaign highlighting the importance of having conversations around healthcare wishes at end of life.
Kelly Baxter holds certifications as a Family Nurse Practitioner, as well as Advanced Certified Hospice and Palliative Care Nurse (ACHPN). She sits on a number of committees including CNE Palliative Care Leadership Team, and Nurses Improving Care for Healthsystem Elders (NICHE) and Chairs the Palliative Care Journal Club and Palliative Care Transitions team.
