Project: "Embedding Advance Care Planning in Care Transitions"
Jannus, Inc.
Boise, ID

Stephanie Bender-Kitz is the Project Lead for Honoring Choices Idaho (HCI), a program of the community-based nonprofit Jannus Inc. HCI, a collaborative of health care, payer, academic and community-based partners, is a major initiative to create a shared model and standard of care that ensures every person’s health care choices are clearly defined, documented, and honored through person-centered advance care planning. Stephanie’s previous leadership roles include building a collaborative community-based network to support and educate chronically ill older adults and family caregivers (Jannus, 2010-2015); serving in a leadership role with the statewide Idaho Caregiver Alliance (Jannus, 2013-present); leading the development of a statewide training program now required of all newly appointed Guardians (Idaho Supreme Court Committee, 2009-2012); and consulting with individuals and organizations to improve support for family caregivers (Kitzworks LLC, 1996-2009).

Participating in the Practice Change Leaders program is a highpoint in a career dedicated to influencing how communities engage, respect and care for older adults and family caregivers. Stephanie brings a combination of an M.S. in Gerontology, M.P.H. with an emphasis in Behavioral Science and Ph.D. in Health Services Research and Policy Analysis to her efforts to build collaboration around a shared vision of health for older persons.
