Peg M. Bradke, RN, MA, is the Vice President of Post-Acute Care at UnityPoint Health - St. Luke’s Hospital in Cedar Rapids, Iowa. She received her Bachelor’s Degree in Nursing from Mount Mercy College and her Master’s Degree in Nursing from the University of Iowa. In her 30-year career, she has had various administrative roles. In her current role, Peg has oversight of a Long-Term Acute Care Hospital, two SNF/ICF nursing facilities, Home Care, Hospice, Palliative Care, Cardiopulmonary Rehabilitation and Home Medical Equipment. Ms. Bradke facilitates the Patient and Family Advisory Council and chairs the Transition to Home Cross-Continuum Team. In addition, Peg served as faculty with the Institute for Healthcare Improvement (IHI) on the Transforming Care at the Bedside (TCAB) Initiative and STAAR (State Action on Avoidable Rehospitalizations Initiative). She continues as adjunct faculty with Institute for Healthcare Improvement. Peg is a 2015 alumuni of the Practice Change Leader Program.
