Angela Lucente-Prokop, MPM, PMP® is the Regional Project Director for Southwestern PA Area Agency on Aging. SWPA Area Agency on Aging, Inc. is a non-profit organization with over 40 years of experience directed towards a mission of advocacy and services to help older adults thrive in their homes and communities. SWPA AAA’s services for older adults and caregivers include care management, service coordination, advocacy, family caregiver support programs, evidence based health and wellness programs and a range of community and social services.

Angela’s current focus is preparing SWPA AAA and 34 of the 52 AAAs in Pennsylvania for Managed Long Term Services and Supports Implementation. She has made significant contributions to the development of service delivery models, cost and return on investment models, quality assurance and performance improvement plans, information systems readiness and training efforts. Angela is also leading NCQA accreditation efforts for SWPA Area Agency on Aging.

Ms. Lucente-Prokop has five years of experience developing and leading Community-based Care Transitions Programs for local Area Agencies on Aging and as faculty and served as faculty chair for the program nationally. These communities represented collaborative efforts to improve transitions in care, reduce avoidable readmissions and improve cost effectiveness across a five-county service area including 2 Area Agencies on Aging, 11 acute care hospital partners, over 24 skilled nursing facilities and additional community partners.

Angela’s background includes over a decade of experience in management and consulting positions including leadership roles at Southwestern PA Area Agency on Aging, Associate Director of Older Adult Services at Family Services of Western Pennsylvania, Management Consultant at The Hill Group, Inc. and Special Projects Manager at Allegheny County Department of Human Services Area Agency on Aging.

She is a graduate of Penn State University and Carnegie Mellon University and maintains credentials in Project Management and Lean Six Sigma. Angela recently completed coursework at the University of Pittsburgh Graduate School of Public Health towards a PhD and certificates in Health Systems Engineering and Health Program Evaluation. Her dissertation focus is evaluating partnerships between community-based organizations and integrated healthcare delivery systems to improve outcomes for vulnerable populations.
