Robert J. Schreiber, M.D., is Medical Director of Primary Care, Community based Programs, Innovation and Development. He is responsible for helping to develop a value based patient directed geriatric medical home. He is engaged in setting the strategic direction of Hebrew SeniorLife (HSL) in home and community based services, long-term care and post acute care. In his role, he is a faculty member of the Institute for Aging Research at Hebrew SeniorLife and is working to connect research to HSL’s clinical venues. In 2010, he was responsible for the Hebrew Rehabilitation Center become a teaching affiliate of Harvard Medical School. He serves as a Clinical Instructor of Medicine at Harvard Medical School.

Dr. Schreiber is a member of the Quality and Patient Safety Committee for the Massachusetts Board of Medicine and has served as an appointed representative for the End of Live Care Commission in the Commonwealth that published recommendations for implementation of palliative care throughout the Commonwealth in 2010. He was awarded the American Medical Director’s Physician of the Year Award in March of 2010.

Dr. Schreiber’s specialty interests include prevention and wellness strategies for older adults. He has been actively engaged in developing new models and approaches to care in various clinical settings. In particular, he has advocated and developed integration of evidence-based approaches to self-management strategies for older adults by connecting the health system and the aging service network provider community in the Commonwealth of Massachusetts and is a principal of the Healthy Living Center of Excellence. He is also a principal in the development and dissemination of the Healthy Eating for Successful Living Program TM which is an evidenced-based Programs to enhance older adults ability to improve their nutritional status and eat a heart and bone healthy diet.
