Cindy Tack is the Director of Care Coordination at the MMC Physician-Hospital Organization in Portland, Maine. In this role she is responsible for providing oversight of and leadership to system-wide care coordination initiatives. Cindy is a social worker by training, receiving her Master’s in Social Work from the University of Wisconsin- Madison in 1987. For the past twenty-five years she has worked as a medical social worker in acute care, rehab and home health care. Prior to her position at the PHO Cindy was the Director of Social Work, Foreign Language Interpreter Services and Sign Language Interpreter Services at Maine Medical Center.

Through her involvement traveling across Maine and the country teaching Stanford’s Chronic Disease Self Management Program, Cindy became interested in adult education and pursued a second Master’s in Medical Education Leadership from the University of New England in 2010. Since then she has served as Adjunct Faculty teaching in their Graduate School of Social Work and currently in their undergraduate College of Health Sciences.

When not at work Cindy enjoys surfing, gardening and haiku.
