Nancy Zaborowski, RN, BSN, joined Advocate Health Care in 2014 and currently serves as Manager, Home-Based Palliative Care, and Manager, Advocate Physicians at Home program. She has been instrumental in developing Physicians at Home, working to create an EMR, establishing program policies and procedures, and developing transition and communication plans to systematically enroll patients. In this role, she also works closely with Advocate Health Care stakeholders and leaders from throughout the system, including those at Advocate Medical Group, Advocate at Home, Advocate Post Acute Network, and Advocate hospitals.

Nancy brought a wide range of experience and expertise to her position at Advocate Health Care. Previous positions include serving as regional vice president of business development for Harden Health Care, Austin, Texas, where her accomplishments included growth in the Medicare market share for home health, hospice and community care in more than 10 states. Past positions also include roles in community relations and discharge planning, along with 8 years as a nurse clinician in the total joint service, Loyola University Medical Center, Maywood, Illinois. She received her bachelor of science in nursing degree from Northern Illinois University, DeKalb.

She resides with her family in Orland Park, Illinois.
